Social Determinants Of Health

Social determinants of health are the conditions in the environment that may have a major impact on peoples’ health, well-being, and quality of life, such as:

  • Safe housing, transportation, and neighborhoods
  • Racism, discrimination, and violence
  • Education, job opportunities, and income
  • Access to nutritious foods and physical activity opportunities
  • Polluted air and water
  • Language and literacy skills

The APhA-APPM Care of Underserved Patients Special Interest Group (SIG) helps identify and periodically reviews the list of resources included on APhA’s Social Determinants of Health webpage. We thank the APhA-APPM Care of Underserved Patients SIG committee members for their contributions.
